About the Role:
Are you a skilled and compassionate Qualified Social Worker with experience in hospital discharge and adult social care? We are looking for an experienced Social Worker to join the Hospital Team in Worthing, West Sussex. In this role, you will work within a fast-paced, multidisciplinary environment, supporting individuals and families through safe and timely discharge from hospital, ensuring continuity of care and effective transition back into the community.

Key Responsibilities:

  • Conduct comprehensive assessments of patients’ needs and create care plans that support safe discharge from hospital settings.

  • Work collaboratively with healthcare staff, family members, and other agencies to develop tailored discharge plans.

  • Assess and coordinate post-discharge services, including home care, residential placements, or reablement services.

  • Support individuals and families to make informed decisions about care options, providing them with clear information and guidance.

  • Liaise with other professionals to facilitate multidisciplinary team meetings and case conferences.

  • Ensure all case records and documentation are accurate and up-to-date in compliance with local authority and regulatory standards.

About You:
To excel in this role, you should be an empathetic and knowledgeable Social Worker with a strong understanding of adult social care, hospital discharge processes, and community-based services. You will be well-versed in managing complex cases and have a proactive approach to facilitating patient-centered care.

Essential Requirements:

  • Qualified Social Worker with active Social Work England registration.

  • Experience in adult social care or hospital discharge planning.

  • Excellent assessment, planning, and organizational skills.

  • Strong knowledge of relevant legislation and best practices in hospital and adult social care.

  • Outstanding communication and interpersonal skills, with the ability to collaborate effectively within a multidisciplinary team.
