Team Manager – Complex Care Team | Hammersmith & Fulham Council | £45 per hour (Locum) | Hybrid Working

Hammersmith & Fulham Council is looking for an experienced Team Manager to join their Complex Care Team on a locum basis, offering a competitive hourly rate and a flexible hybrid working model.

This is a fantastic opportunity for a strong leader to oversee a busy adult social care team supporting individuals with complex needs, ensuring high-quality practice and positive outcomes across the service.

About the Role

As Team Manager within the Complex Care Team, you will provide operational leadership and management oversight to practitioners working with adults requiring complex interventions and long-term support.

Key Responsibilities:

  • Lead and supervise a team managing complex adult social care cases
  • Oversee assessments, reviews, care planning, and safeguarding interventions
  • Ensure compliance with the Care Act and statutory responsibilities
  • Support practitioners with complex decision-making and risk management
  • Monitor team performance, quality assurance, and service delivery
  • Work collaboratively with health professionals and multi-agency partners

About You

We are looking for an experienced and motivated manager with a strong background in Adult Social Care and complex case management.

Requirements:

  • Qualified Social Worker with Social Work England registration
  • Previous Team Manager experience within Adult Social Care
  • Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures
  • Experience managing complex and high-risk cases
  • Excellent leadership, organisational, and communication skills
  • Ability to support and develop staff within a fast-paced environment
